Service Agreement Details
This service agreement is between the NDIS participant or participant's representative within NDIS and Thrive Ed Co and will commence on the date signed.
NDIS and this Service Agreement
This Service Agreement is made for the sole purpose of providing supports to the participant under their NDIS Plan
The Parties:
1. NDIS Participant/Participants Representative (Named above)
2. Thrive Ed Co
Agree that this service agreement is made in the context of the NDIS which is a scheme that aims to:
+ Support the independence and social and economic client of people with disability;
+ and enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.
Schedule of Supports
Thrive Ed Co agrees to provide the participant Occupational Therapy Services. (A detailed description of the services, sessions length, frequency and fees are listed further in this agreement)
All prices are GST inclusive (if applicable) and include the cost of providing the supports. Additional expenses (i.e. things that are not included as part of a client NDIS supports) are the responsibility of the participant/participants representative and are not included in the cost of the supports.
Thrive Ed Co Responsibilities
Thrive Ed Co agrees to:
+ Review the provision of supports annually with the client;
+ Once agreed, provide supports that meet the client's needs at the client's preferred times;
+ Communicate openly and honestly in a timely manner;
+ Treat the client with courtesy and respect;
+ Consult the client on decisions about how supports are provided;
+ Give the client information about managing any complaints or disagreements and details of the Thrive Ed Co cancellation policy;
+ Listen to the client’s feedback and resolve problems quickly;
+ Give the client a minimum of 24 hours notice if Thrive Ed Co has to change a scheduled appointment to provide supports;
+ Give the client the required notice if Thrive Ed Co needs to end this Service Agreement;
+ Protect the client's privacy and confidential information;
+ Provide supports in a manner consistent with all relevant laws, including the NDIS Act 2013 and Rules, and the Australian Consumer Law;
+ Keep accurate records on the supports provided to the client; and
+ Will issue invoices and statements of the supports delivered to the participant as per the NDIA Terms of Business for Registered Providers as requested
Participant/Participant's Representative Responsibilities
The Participant / Participants’ representative agrees to:
+ Inform Thrive Ed Co about how they wish the supports to be delivered to meet their needs;
+ Treat Thrive Ed Co workers with courtesy and respect;
+ Talk to Thrive Ed Co if they have any concerns about the supports being provided;
+ Give Thrive Ed Co the required notice if they cannot make a scheduled appointment, noting that if the notice is not provided, Thrive Ed Co cancellation policy will apply;
+ Give Thrive Ed Co the required notice if the participant needs to end this Service Agreement
+ Let Thrive Ed Co know immediately if the NDIS plan is suspended or replaced by a new NDIS plan, or the participant stops being a participant in the NDIS.
SELF MANAGED
The participant/participant's representative has chosen to self-manage the NDIS funding. Thrive Ed Co will seek payment for the services provided at the end of the session. If an agreement has been made between Thrive Ed Co Therapy and the participant/participant's representative an invoice will be issued to the participant/participant's representative for payment within 48hours.
Changes to this Service Agreement
Thrive Ed Co will review this agreement annually. If there are changes regarding the supports required or the delivery of supports, the parties agree to discuss and review this agreement. The parties agree that any changed made to this service agreement are to be in writing, signed and dated by both parties.
Ending this Service Agreement
If for any reason either party wish to end this service agreement they must provide at least two weeks’ notice in writing.
If for any reason either party breaches this service agreement the notice period will be waived.

Goods and Services Tax (GST)
For the purposes of GST legislation, the Parties confirm that:
+ A supply of supports under this Service Agreement is a supply of one or more of the reasonable and necessary supports specified in the statement included, under subsection 33(2) of the NDIS Act, in the client's NDIS Plan currently in effect under section 37 of the NDIS Act;
+ The participants NDIS Plan is expected to remain in effect during the period the supports are provided; and
+ The participant/participant's representative will immediately notify the provider if the client's NDIS Plan is replaced by a new plan or the client stops being a participant in the NDIS.
Appointment Cancellation Policy
Thrive Ed Co works with the same families throughout the year at regularly scheduled appointment times, so it is often difficult to fill appointment sessions when they are cancelled, especially at the last minute
Late notice cancellation is when a client cancels and fails to provide 48 hours of their scheduled appointment.
Failure to attend is when a client does not attend their scheduled appointment and has not provided notice of cancellation.
In both instances, you will incur a fee:
Cancellations within 48 hours of your appointment time - 50% of the session fee
Cancellations within 24 hours of your appointment time - 100% of the session fee
Failure to attend your appointment - 100% of the session fee
Outline of Your Schedule of Supports
I/we agree to pay for literacy support provided by Thrive Ed Co through claiming against the National Disability Insurance Scheme (NDIS) service plan
The Provider agrees to provide the client literacy services for the duration of the agreement at the scheduled rate. These include
+ Literacy diagnostic and treatment
+ Clinically relevant communications including phone calls/written programs/communication with other health professionals (any task that takes more than 30 minutes will be invoiced)
+ Attendance at team meetings/case conferences including virtual attendances when required
+ Any reports, forms or letters as required by the NDIS or requested by the participant/participant's representative
+ Cancellation charges for late notice or no show appointments
Thrive Ed Co reserves the right NOT to provide service or to cancel any future appointments for the client if you do not have sufficient funds in your plan or the plan expires. Any service fees not met by NDIS will be covered by the participant/participant's representative.
End of Plan NDIS Reports
A plan review is an opportunity for participants to look at their progress and set new goals to increase their skills and independence. A report we provide will provide evidence of the outcomes achieved and the progress made towards your goals.
End of plan reports are $175 + GST per hour, which is not included in the fees above. Reports can take 2-6 hours to prepare, please ensure you have funding allowance for a report, before requesting a report from Thrive Ed Co. Reports will not be released to the participant/participant's representative until payment has been made.
To ensure that a report from Thrive Ed Co is available for your plan review please provide one months notice prior to your plan review date.
Travel for off-site Education Session
Home, Day Care and School environments are off site sessions.
Thrive Ed Co WILL NOT be charging the extra $ 0.85 per km
These sessions will only incur an additional travel fee which will be charged to your NDIS funding.

NDIS fee updates
All prices will be adjusted if there is any change in the NDIS price guide during the service agreement period. Any changes will be discussed, and updates made in a new service agreement to be signed by both parties.
